Evidence of disability

If you are applying for supports/reasonable accommodations from the MTU Cork Disability Support Service (DSS) you need to provide evidence of your learning difference, health condition or disability. 

 

This can be in the form of documents giving diagnosis details; impact of your learning difference, health condition or disability; or details of related supports in previous education. 

If you have already submitted your documents to the CAO/DARE, you do not need to submit another copy unless you have new or updated information.

 

Examples of evidence:

  • Diagnosis report from a relevant professional (see this document for more details). 
  • Letter/statement from previous education outlining information and /or supports related to your learning difference, disability or health condition, such as a State Examinations Commission letter regarding Reasonable Accommodations at the Certificate Examinations (RACE), or DARE Educational Impact Statement.
  • OR if you do not have these available you can ask a GP, Health Professional /or Specialist supporting you to provide details related to your learning difference, disability or health condition and impact of this by completing this Evidence of disability form.

How to do I send my evidence of disability to the DSS?

For paper copies, you should scan in or take good-quality photos of each page of the documents. You can e-mail them to dssCork@mtu.ie  (total size of attachments no larger than 10MB per email). The DSS email is a confidential email.
